Expressing Your Wishes and Preferences

Health care directive forms provide space for you to tell about your wishes and preferences for care.  The information you put in this part of the form can help your agent(s), your loved ones, and other care providers understand what you want and guide their decisions about your care. 

Some people use this part of the directive to explain their idea of what makes life worth living, their religious or spiritual beliefs about life and death, their fears about dying or hopes for a “good death.”  Some people include their feelings about specific medical treatments such as pain control, nutrition support (feeding tubes), and cardiopulmonary resuscitation (CPR).
